Cryptosporidium oocyst: modified AFB vs. yeast comparator
Oocysts stain pink-red (acid-fast positive) with variable intensity against malachite green. About 80% are thick-walled (passed in stool); about 20% are thin-walled (cause autoinfection). Yeast co-occurs and stays green.
4-6 um round. Modified AFB: pink-red staining varies from bright to faint "ghost" on the same slide - variable intensity is expected. Contains 4 sporozoites; thick-walled (80%) exit in feces, thin-walled (20%) cause autoinfection.
3-8 um, non-acid-fast: retains green counterstain. May bud or form pseudohyphae. A slide with only green structures = no Cryptosporidium.

What to look for on modified acid-fast stain

- Modified AFB staining intensity is variable - scan the full slide; don't rely on bright-pink structures only.
- Thick-walled oocysts (80%) leave in stool -> infect new hosts. Thin-walled (20%) excyst in intestine -> autoinfection in immunocompromised patients.
- IFA and multiplex GI PCR panels are substantially more sensitive than modified AFB for low-burden or equivocal specimens.
4-6 um + any pink (even faint) + no budding = Cryptosporidium. Green = yeast. Variable staining on the same slide is expected.
